Fantasy Alive Age Restriction

Fantasy Alive is a game for adults ages 18 and up. 

Potential players ages 15-17 may also participate in the game so long as all aspects of this form are agreed upon by the underage player, their legal guardian and an adult guardian who must attend the event with the underage player.

Some special Fantasy Alive events will be all-ages, and will be properly advertised as such with any restrictions required.
